Opioid Resources
Disease Management
Members living with a chronic condition may benefit from participating in a Blue Care Connection® Disease Management program. These comprehensive health management programs are designed to help members understand the day-to-day management of their condition, support their provider's plan of care, and improve their quality of life. The programs provide individual self-assessment and educational tools, and when appropriate, support by phone that enables members to take a more active role in their own health management.
Case Management
We offer specialized support services for certain conditions through our comprehensive group of Blue Care Connection Case Management programs.
Case Management programs are designed to support individuals with more complex care needs associated with severe illness, injury, or other conditions. Participants are identified for the Case Management program through a combination of claims data, physician referral, and self-referral. A specially trained registered nurse offers specialized support for each Blue Care Connection program.
Advance Care Planning
We want our members to get the care they want and need, at every stage of their lives. Conversations about patients' wishes for end-of-life care — a process known as Advance Care Planning — can be an important step toward that goal. These tools will help guide your employees through the important work of Advance Care Planning.
